Eligibility

Wyeth Removal Certificate, a patient assistance program provided by Wyeth Pharmaceuticals, offers the free removal of the I.U.D. to those who are eligible for the program. Eligibility is based off of the following requirements:

- You must not be covered by private or public health insurance.

- The I.U.D. must have been installed and must be removed within the United States.

- Your income must be less than or equal to 200% of the Federal Poverty Guideline for the size of your household (see chart below).

You must contact the company for a phone based prescreening.

This program only pays for the removal of the I.U.D.. No other costs or fees are covered.
