Eligibility:
AZ&Me Prescription Savings Program For People With Medicare Part D, a patient assistance program provided by AstraZeneca Pharmaceuticals, offers the medications listed to the right at $25 for a 30 day supply to those who are eligible for the program. Eligibility is based off of the following requirements:
- You must be enrolled in a Medicare Part D Plan.
- Your annual income must be no greater than $30,000 if single, but no greater than $40,000 for your entire household.
The Patient is sent an enrollment kit and a program card
